I just got my feedback from Ms Chan regarding my fracture practical exam and it didn't turn out well.
After the exam i thought i was doing fine but well, it wasn't and it was pretty wrong. It was an easy question but i screwed it up. What can i say about my foolishness.
So Ms Chan told me that there has no stabilization while doing joint mobilization on pillows. One. Secondly, doing joint mobilization in sitting position is very hard for the therapist. He/she may may need to give more energy to mobilize the joint and end of the day he/she will have problem with him/herself. Then, exercise prescription. Using weight to do pronation and supination exercise is off. I did not take patient's recovery time into consideration. Normal cortical bond takes about 6 weeks to recover or becomes stable. In my question my patient was immobilized for 4 weeks and consideration on this should be there. 4 weeks, no too strenous exercise and joint mob needs to be done with VERY GOOD support. It was just my luck saying i'll rather prefer doorknob instead of weight since doorknob equipment is not available there. Plainly luck.
She said she remembered crystal clear about my positioning, especially my positioning because it was a blow in her mind. LOL! Anyway, i managed to pass but not with a very high marks.
2 things I have to remember forever while dealing with fracture:
- Take healing period into consideration while prescribing an exercise.
- Joint mob needs proper positioning and stabilization. Especially stabilization.
I thought this is gonna be fine but it went the other way round. May I remember this forever and ever along my physical therapy journey.
